To Whom It May Concern:

Pursuant to the Rhode Island Access to Public Records Act, I hereby request the following records:

Copies of any contractual agreements between the "Equity Institute" and the Portsmouth School Department.

Copies of any and all surveys intended to be used by the Portsmouth School Department during the equity analysis currently being undertaken. Surveys are to include but are not limited to, surveys prepared for students, parents, teachers, principals, administrators, employees, staff members, school committee members, or any other survey prepared to be used by the Portsmouth School Department for the equity analysis.

Copies of any and all literature, written materials, and the like, supplied to the Portsmouth School Department by the Equity Institute, to include any and all literature/materials intended to be used for the equity analysis. Additionally, please be sure to include any literature/materials intended to be used by any focus group.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
