Pursuant to the Virginia Freedom of Information Act, I hereby request the following records:
- A copy of any data release consent forms parents or students are asked to sign
- Any current policy documents or guidelines governing student data usage
- Any amendments made to those forms from 2015-2019
- Documents outlining any third-party programs or applications collecting student data
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Good afternoon, Mr. Nash
This letter is to acknowledge your request for information dated Feb. 10
sent via fax and received in my office that same day.
You requested the following:
1. A copy of any data release consent forms parents or students are asked
to sign
2. Any current policy documents or guidelines governing student data usage
3. Any amendments made to those forms from 2015-2019
4. Documents outlining any third-party programs or applications collecting
student data
For item one, attached you will find two forms: the division’s Media and
Directory Information Opt-Out Form as well as the division’s Acceptable Use
Agreement.
For item two, the division follows the national standards outlined in the
Family Educational Rights and Privacy Act as well as the Public Schools of
Virginia policy on the management of student data. Beyond that, attached
you will find policy *GAB/IIBEA Acceptable Computer System* use as well as
regulation *IIBEA-R/GAB-R Technology Use Guidelines*.
For item three, the division has attached the previous two years of the
Media and Directory Information Opt-Out Form as these are the only records
kept. There had been no change to the Acceptable Use Agreement in the past
five years.
For number four, attached you will find three spreadsheets for application
account management, which outlines the programs or applications collecting
student data, as well as the Clever Applications List and its specific
field mappings.
Finally, please note our offices were closed Monday, Feb. 17, in observance
of Presidents’ Day. In addition, the email address for FOIA requests is
