Request for Elmwood Park Fire Department EMS/Ambulance Transports

To Whom It May Concern:

Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:

Please provide all records pertaining to the Village of Elmwood Park's Fire Department for EMS/ambulance transports (completed transports) provided from May 1, 2020 to April 30, 2021. The report needs to include the following data points at a minimum:

1. Incident Date
2. Incident Number
3. Location (Address)
4. Brief Description of Incident (Nature Code will suffice)
5. Indicate whether or not an ambulance transport was made or not.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
