Pursuant to the Oregon Public Records Law, I hereby request the following documents:

* Any policies and procedures for storing, handling, and maintaining evidence

* Any policies and procedures for conducting audits of evidence

* All audits of evidence that have been conducted between January 1, 2012 and the present; if no audits occurred during this period, then please provide the most recent audit that was conducted prior to 2012

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I would request your response within ten (10) business days.

Note: If you pay in person, our office hours are from 8:00am to noon and 1:00pm to 5:00pm. We are closed from noon to 1:00pm.

Once payment has been received, staff will produce the records and make copies, and I will review the information. If the records are copyrighted, the City will require you to complete a release form and/or obtain permission from the copyright holder(s) before documents can be released. I will email you if this applies to your request.

If the deposit amount is greater than the actual fees, a refund will be issued to you. If the deposit amount does not cover actual fees, you will be requested to submit additional funds.
