1. Please provide an export of all data from the HSH One system, the Bitfocus Clarity Human Services Homeless Management Information System (HMIS), including the HUD CSV/XML Program Data Export. Provide the data in .CSV format.
2. Please provide a breakdown of all services offered for each person listed in the "Clients Served" section of this report and any data from the HMIS relevant to the individuals listed in this report: https://sanfrancisco.nextrequest.com/documents/7988350/
Please provide results on a rolling basis.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Please note that we are invoking an extension of time to continue our response for up to 14 days under Government Code § 6253(c) and San Francisco Admin. Code § 67.25(b) in order to consult with another department or agency that has a substantial interest in the response to the request. As well as to construct a computer report to extract the most up-to-date data. Due to the extension, we will respond by COB, on or before Tuesday, August 24, 2021.

Note: Column G refers to when a client is linked to a temporary shelter option run by HSH. It also refers to linkages to programs that are run by DPH (i.e. substance use treatment programs and mental health services). Column H refers to when clients are escorted to the ER for any reason (i.e. mental health or medical help).

Hi, thank you for the response. The data dictionary that was sent is very limited and appears to only include fields from the "SFHOT Current Living Situation/ Encounter Form". Can you please provide the COMPLETE data dictionary for the entire ONE system? For example, the report you supplied for the breakdown of services includes date and location fields but the data dictionary does not. The dictionary also does not include race or age. Please provide the entire dictionary. Thanks.

Apologies: we lost some formatting when sharing this back with you. We are sharing data on all the custom fields that we have in ONE in the attached Excel file. These are fields that are used for specific program and assessment elements in SF. This includes data on :
1. Custom fields in the Client Profile Page
2. The Family Eligibility Assessment
3. Veteran Outreach Eligibility Assessment
4. Navigation Center Case Management
5. Family Shelter Assessment
6. Family Housing Prioritization Assessment
7. Adult CE Assessment
8. Stabilization Room custom elements
9. FEMA NCS elements
10. Pandemic Prioritization Assessments
11. Posting forms for vacancies
Additionally, ONE implements all mandatory HUD HMIS elements (currently, the 2020 standards attached as PDF). These elements are used in all our templates for all of HSH’s programs. HUD specifies what should be collected for all demographic and program enrollment elements in the attached Data Standards file. These are applied to all program types we are funded for including shelter, street outreach, temporary housing and permanent supportive housing using HUD guidelines. Data on Race, Ethnicity etc are implemented in ONE per the HUD guidance for Universal Data Elements.
