Records of any and all CMS Form 2567s on file for the following home health and hospice agencies for the inclusive period between January 1, 2015 and September 1, 2020.( I have already reached out to CMS in this regard and they have suggested requesting these forms from the state health agencies.)
In the event that there are multiple agencies with the same name or with almost identical names, I intend this request to encompass all those agencies. LHC Group is a nationwide company that operates the other home care agencies as subsidiaries in the state of Tennessee.

Good morning Mr. Hoover.
We have provided an estimated cost for the labor dedicated to collecting and producing these records. The Public Records Act states, "If a person makes a request for copies of a public record and, after copies have been produced, the person fails to pay to the governmental entity the cost for producing such copies, the governmental entity is not required to comply with any public records request from the person until the person pays for such copies; provided, that the person was provided with an estimated cost for producing the copies in accordance with subdivision (a)(7)(C)(ii) prior to producing the copies and the person agreed to pay the estimated cost for such copies." T.C.A. 10-7-503 (a)(7)(A)(vii)(b).
You received an invoice on October 29, 2020, and the department proceeded with your request. At this time, no payment has been received for copies of the requested records. Payment is required before they are released to you. Please send your payment to the address provided on the invoice.
If you have changed your mind about this request, please respond to this email and withdraw the request.
