Pursuant to the Tennessee Public Records Act, I hereby request the following records:
I seek information related to the Tennessee Employment and Case Management Services solicitation (GFA #34530-45720). The release date for the solicitation was April 18, 2019, the due date was May 3, 2019, and the award date was May 21, 2019. The contact person for this bid is Karen Walker, Director, Family Assistance Contracts Tennessee . Ms. Walker can be reached at karen.x.walker@tn.gov or (615) 253-4313.
Specifically I seek copies of the following information:
- The winning proposal (cost and technical)
- All competitor proposals (cost and technical)
- All invoices associated with this contract from 12/18 on for all vendors
- All compliance reports associated with this contract from 2018 on for all vendors, including performance reviews and audits
- Award evaluations/score sheets and any evaluator notes for all vendors
- The contract and any amendments since 2018
- All caseload data for April 2018-July 2018 and April 2019-present.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 7 business days, as the statute requires.
