Autopsy report pertaining to cause of death as well as toxicology report for Angela Fresquez .
Date of birth was August 31, 1987. Date of death was January 29, 2022.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

We do not accept personal checks, cash or credit cards.
Please include our case number or the decedent's name, your mailing address, and signature on money order or certified check.
We will consider your request automatically withdrawn if you do not respond in writing to this notice and itemization of charges within ten business days from the date of this notice. Please advise us in your response if you accept the charges or are modifying your request.
