To whom it may concern:

I have not submitted any original documents USCIS and I am not sure if G-884 form solves the purpose in this case. My employer has filed for an I-140 (Immigration petition for alien worker) on my behalf and was approved by USCIS (Receipt Number: SRC1520050163). But, my employer has not shared a copy of the I-140 approval letter with me. I have submitted a form G-639 for this purpose and mailed to the following address on the 3rd of August 2015 and have not received a receipt number yet:

U.S. Citizenship and Immigration Services
National Records Center, FOIA/PA Office
P. O. Box 648010
Lee’s Summit, MO 64064-8010.

Please let me know if I still have to submit the form G-884. I will be happy to submit the form.

Sincerely,
Mrutyumjai Kamana
