Pursuant to the Freedom of Information Act, I hereby request the following records:
I would like to receive an electronic copy of all documentation pertaining to any and all background checks, Periodic Reinvestigation(s) for Security Clearance, and any other documents or forms pertaining to these between 11/1/2015 and 11/1/2021.
I am submitting this under the Freedom of information ACT, as these documents pertain to me.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not. I am also willing to Inspect these documents, please contact me at the number provided below if this arrangement is a viable alternative.

On September 30, 2019, as authorized by Executive Order 13869, the mission, records, and personnel of the U.S. Office of Personnel Management (OPM), National Background Investigations Bureau (NBIB) transferred to the Department of Defense, Defense Counterintelligence and Security Agency (DCSA).
Accordingly, as of October 1, 2019, DCSA's FOIA and Privacy Office for Investigations is primarily responsible for responding to requests for information surrounding the investigative process for civilian and military applicants, Federal employees, or employees or government contractors for the purpose of determining suitability for government employment, fitness for appointment to an expected service position, acceptance or retention in the armed forces, eligibility to serve in a national security sensitive position, eligibility for access to classified information and/or eligibility for access to classified information controlled facility or information technology system.
