This is a request under the Freedom of Information Act. I hereby request the following records:

Copies of all memoranda, reports, presentations, and emails from and to USAMRMC and subordinate activities including USAMMA from and to any representatives of the Defence Health Agency, the U.S. Army, U.S. Navy, U.S. Air Force, U.S. Marine Corps, and the Combatant Commands including USAFRICOM, USCENTCOM, USINDOPACOM, and USSOUTHCOM, related to proposed or planned policy for the use of tafenoquine (Arakoda) in the chemoprophylaxis of malaria.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

On December 11, 2018, I submitted a request under the FOIA for "[c]opies of all memoranda, reports, presentations, and emails from and to USAMRMC and subordinate activities including USAMMA from and to any representatives of the Defence Health Agency, the U.S. Army, U.S. Navy, U.S. Air Force, U.S. Marine Corps, and the Combatant Commands including USAFRICOM, USCENTCOM, USINDOPACOM, and USSOUTHCOM, related to proposed or planned policy for the use of tafenoquine (Arakoda) in the chemoprophylaxis of malaria".

In response, your office assigned this request tracking number FP-19-0009.

As of today, I have yet to receive a response to this request.

There seems to be confusion in the numbers for the FOIA Requests. FA-19-0009 was referred to MEDCOM for final processing. Mr. Peterson sent you a SAFE link with those documents, which is the request you are referring to below in the email.

FA-19-0010 - Was the FOIA request that you appealed, which was the request for copies of purchase orders, contracts, records, and related correspondence between USAMMDA and sub activities of MRMC, 60 degrees pharmaceuticals or any wholesale or distributor for supplies of TQ intended for military use from 2013-present. MEDCOM, Mr. Peterson was working on the appeal with you. Our USAMRAA Office will be sending you additional documents related to this request as well.
