This is a request under the Freedom of Information Act. I hereby request the following records:

All releasable information from the Official Military Personnel File (OMPF) of Joseph Cuellar including, but not limited to, fitness reports, counseling forms, training records, background summary, Officer Personnel Action Requests and Form DD-214.

Proof of death is not required as his death has been widely and reliably reported. He is buried in Arlington National Cemetery. http://apps.westpointaog.org/Memorials/Article/34440/

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Form 180 and submit it since only the name of the Veteran was provided and
a identification of the record is not possible with such limited
information. The information you are requesting is not releasable under the
Freedom of Information Act, only those things covered under FOIA are
releasable, for example, Name, Branch and Dates of Service, Training,
Awards and Medals, Assignments, Rank, Photograph and Place of Entry and
Release are releasable. If you are interested in obtaining those things,
