DATA waivered practitioner patients

To Whom It May Concern:

Pursuant to the Freedom of Information Act, I hereby request the following records:

A copy of any spreadsheet or other document that includes the number of patients each DATA waivered prescriber may treat. I am aware of the practitioner finder database on your website, however it does not include the number of patients each of these practitioners may prescribe to, which is what I am seeking. If such information is not readily available, then any spreadsheet or document that lists which physicians have been waivered to prescribe to 30 patients, 100 patients, and 275 patients, will suffice. In addition, if you have data regarding how many patients each prescriber actually treats, that would be extremely useful as well.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
