Pursuant to the Freedom of Information Act, I hereby request the following records:
The Official Military Personnel File (OMPF) concerning Donald C Hubbard including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2, NAVPERS form 1610/1,, NAVPERS Form 1611, Administrative Remarks Navpers 1070/613, Background Summary (NAVPERS Form 1640/32), Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS 1070/605, Navy Training History NAVPERS form 601-4, all military decoration and award citations, all military education, military service number.
Mr Donald C Hubbard claimed service in United States Navy as a commissioned officer from approximately 1979–1990 active duty. Graduated United States Naval Academy class of 1979.
Claims awarded many decorations including meritorious service medal, Navy commendation medal, navy achievement medal, Navy Unit Commendation and others. Specifically request copy of all personal commendation medal citations and unit commendation citations with redactions if necessary.
NPRC initial search claims unable to locate the OMPF file, advised to contact Navy Personnel Command for more information such as military service number or NPRC registry number. Please assist in finding the records.
I represent the general public and thus do not know the military service number. I also request that, if appropriate, fees be waived as I believe this request is in the public interest . His background military experience and qualifications are a matter of public interest. Mr Hubbard was management leader of technology investment banking for Europe with Morgan Stanley and previously managing director with Merriman Curhan Ford & Co, currently executive with Barrings Capital. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press .

Hello,
May we request the military service number of subject servicemember on file so we can refile with NPRC again.
NPRC still claims they cannot find the OMPF file even after provided their own registry Number 000009048443 from the US Navy Personnel Command which transferred the file to them. We believe NPRC is unable to perform searches and locate records without the military service number. The records are confirmed located at National Personnel Records Center in St. Louis . Mr Donald C Hubbard served as commissioned officer from approximately 1979–1990 active duty. Graduated United States Naval Academy class of 1979.
