The Official Military Personnel File (OMPF) including all Fitness Reports and Counseling forms (NAVPERS Form 1610/2, NAVPERS 1610/1, NAVPERS 1611-1 Eval/Fitreps), history of assignments (NAVPERS Form 1070/605), the most recent Cumulative Record of Training (NAVPERS Form 5300/16), the Background Summary (NAVPERS Form 1640/32), form NAVPERS 1070/884 final active duty officer photograph, Administrative Remarks NAVPERS form 1070/613, military awards and full citations, military education/training courses and Form DD-214, for TIMOTHY LEE BOSILJEVAC. Please redact any sensitive information protected in accordance with the Privacy Act of 1974. please provide all requested documents.
Mr Tim L Bosiljevac has claimed publicly to have served in the United States Navy as a commissioned officer rank O-6 - Commander, approximate dates of navy military service (years 1981 to 1999). Born approximately 1953-1956. Claims decorations including Joint Service Commendation Medal, Joint Meritorious Unit Award, and numerous other commendation medals and unit awards. Request copy of all award citations with or without redactions. There is a need to inform the public of his military background experience.
I also request that fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com.
