The Official Military Personnel File (OMPF) for Timothy Paul Prusak (born January 18, 1950) including all Fitness Report and Counseling forms NAVPERS Form 1610/2, Enlisted performance evaluation reports NAVPERS Form 1616-1, history of assignments NAVPERS form 601-5 and/or NAVPERS form 1070/605, the Background Summary (NAVPERS Form 1640/32), Navy Training History NAVPERS form 601-4, Most recent Cumulative Record of Training (NAVPERS Form 5300/16), all military awards and full citations, military education/training courses
and Form DD-214.
