Pursuant to the Freedom of Information Act, I hereby request the following records:
A copy of all documents, including but not limited to, applications, agreements, and reporting documents, including all annual reports and other reporting materials submitted by the grantee for Award # 15PBJA-23-GG-00091-BRND awarded to McDermott Center Inc. for the following purpose: "The purpose of Haymarket Center’s Project Restart is to reduce recidivism among as many as 200 adults who are transitioning from inpatient treatment for substance use disorder to communities in the service area of Chicago’s south and west sides but are at risk of recidivating. Project Restart recognizes that recidivism is not caused by just one or two factors, but rather by a host of factors demanding tightly coordinated services from professionals and peers alike. "
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
