Pursuant to the Freedom of Information Act, I hereby request the following records:

Copies of all regulations, recommendations, guidance, or other advice to state, municipal or local governments, public or private hospitals, care facilities, prisons, or other bodies, on what information and data they should make directly available to the public and media (including definitions and formats) regarding COVID-19 including known and suspected cases, deaths, hospitalizations, availability of health care facilities including beds and ventilators, emergency plans, contract tracing, and algorithms, models, and underlying assumptions used to estimate spread of disease.

If guidance is not available for COVID-19 specifically, any similar general guidance or guidance especially written for other diseases including Ebola, Legionnaires, SARS, H1N1, MERS, and Zika.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
