This is a request under the Freedom of Information Act. I hereby request the following records:

1) A copy of the "Detailed Budget and Justification" form submitted as an attachment for Proposal Number GW093066 under the Gulf War Illness Veterans Research Program (GWIRP) study by Principal Investigator Dr. David Carpenter, State University of New York Albany (Grant Award Number: W81XWH-10-1-1004) that was not included in my previous FOIA Request Case# FP-15-021393/FA-15-003.

2) A copy of the revised "Detailed Budget and Justification" form submitted as an attachment for the "No Cost Extension on award W81XWH-10-1-1004" letter from the State University of New York Research Foundation (SUNY RF) dated January 7, 2014 that was not included with the disclosure of said letter released in response to another one of my previous FOIA Requests on December 21, 2015 (No FOIA Case# provided).

3) A copy of the revised "BIOGRAPHICAL SKETCH" form for Dr. Gayle Morse submitted as an attachment for the "No Cost Extension on award W81XWH-10-1-1004" letter from the State University of New York Research Foundation (SUNY RF) dated January 7, 2014 that was not included with the disclosure of said letter released in response to another one of my previous FOIA Requests on December 21, 2015 (No FOIA Case# provided).

I also request that, if appropriate, fees be waived as I believe this request is in the public interest due to the recent information widely reported on about this ITEA grant funding a clinical trial on a controversial Scientology detoxification program. For more information, please see:

Additionally, requested documents will be made available to the general public, and this request is not being made for commercial purposes.
