Pursuant to the Freedom of Information Act, I hereby request the following records:

I am requesting a log of 9-1-1 calls categorized as a mental health issue from January 2015 to the present. I expect this log would include the following information:

-Event or CAD identification number
-Date of call, time of call and location of call
-Description of event
-Response sent, disposition (including arrest, hospitalization or other outcomes)
-Incident data, and any other result of service.

Please provide the response in a csv format. I am NOT seeking names of individuals or other information that might violate privacy protections. I am NOT seeking patient examination or treatment information. I am simply seeking records sufficient to disclose 911 emergency calls categorized as mental health.

I am also requesting a listing, dictionary or glossary that notes the definition of any numerical or alphabetical codes used to designate types of emergency calls (for example, EDP for Emotionally Disturbed Person.)

I’m making this request as a journalism student at Northwestern University working on a project for publication with MindSite News, a not-for-profit journalism organization that works with students and partner news organizations to gather, study and disseminate data. I hereby request a fee waiver, as the request is being made for non-commercial purposes with the intention of journalism research in the public interest. If a fee waiver is not granted, I am willing to pay fees for this request up to a maximum of $10. If you estimate that the fees will exceed this limit, please inform me first. Please let me know promptly if there are going to be any delays beyond the statutory deadlines. Because we are a journalism organization working in the public interest of PR citizens, I appreciate anything you can do to expedite this request.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
