The USDA’s Animal and Plant Health Inspection Service (APHIS) has asked anyone who receives an unsolicited package of seeds to complete the form below:
I'm seeking copies of the responses submitted through the "Unsolicited Seeds Self-Reporting Form."
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

Thank you for your patience and understanding with the process thus far and please note that after receiving some information from the program office responsible for conducting the search for records, it appears that it is going to take some time to complete the search. The estimate we currently have is that the search should be completed no later than 26 February. Depending on the amount of records found and how complicated the review of the records may be, I am estimating that March 12, 2021, is when you are likely to receive a response from us regarding your request.
