Need Information on Case with Homeland Security

To Whom It May Concern:

This is a request under the Freedom of Information Act. I hereby request the following records:

My friend and I met with Special Agent Mike Perez and another agent with Homeland Security for Middle Tennessee on November 16, 2016. It was regarding a complaint by me against a man named Adam Shoulders in Nashville, TN. Adam claimed to be working with and for DHS. I told Agent Perez and the other agent that they could contact my friend, Lee Matous about the case. However, I no longer trust Mr. Matous and I never heard the result of that meeting. I no longer have Agent Perez' card and cannot find his number from calling DHS. I would request that Agent Perez contact me by telephone at 615.979.2210 or email at janetwyatt2003@yahoo.com regarding this complaint.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
