All complaints filed about Mallinckrodt Pharmaceuticals and Covidien since 1996.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Good afternoon Ms. Moore,

Apologies for the delay in response. I was out of the office for several weeks recently.

With regards to complaints, OSHA's disposition schedule for invalid complaints (no OSHA jurisdiction, Items not covered by OSHA, items not described in enough detail to act, exposure beyond OSHA statue of limitations) is 1 year and for valid complaints is 3 years, per OSHA Directive Number: ADM 03-01-005

All complaints at this facility were beyond these disposition dates.

There are no current complaints.
