The University has determined that disclosure of certain information contained in the requested records, such as personal contact information, patient names, and other private details, would constitute a clearly unwarranted invasion of personal privacy. Thompson, Records Officer, J. Willard Marriott Library, 295 South 1500 East, Salt Lake City, Utah, 84112, who is authorized to hear your appeal pursuant to Utah Code § 63G-2-401(9). A notice of appeal must be submitted within thirty (30) days of this denial, and must include your name, mailing address, daytime telephone number, and statement of the relief sought. It may also include any facts, reasons, or legal support for the appeal.
