Pursuant to the Vermont Public Records Act, I hereby request the following records:

Historical licensing data for all licensed child care providers (active and closed) that were open anytime from 1/1/19-present:
1) Provider name or ID
2) Provider address
3) Latest license issue date and expiration date
4) First license issue date
5) Enrollment, including dates on which enrollment was recorded (by child age groups if available)
6) Capacity, including dates on which capacity was recorded (by child age groups if available)
7) Prices, including dates on which prices were recorded (by child age groups if available)
8) Quality Rating and Improvement System (QRIS) scores, including dates on which scores were recorded
9) Any additional provider data that your agency collects

If electronic records from before 1/1/19 are available, I’d be happy to collect them as well.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Unfortunately, we have not yet received your check here at the Department for Children and Families. As you know, sometimes around the holidays the mail is delayed. I will keep my eyes open for it. Or, if you prefer, you can research on your end and make sure that the check has not been cleared and void and reissue another one.
