Pursuant to the Vermont public records law (1 V.S.A. §§ 315-320), I hereby request the following records:
Any and all documents related to applications approved or denied submitted to the State of Vermont for operation of clinics providing abortion services.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

I have responded to this request a few times. I am not sure if you are receiving these emails. We have no records that are responsive to this request. We do not handle applications for any facilities in the Central Office of the Agency of Human Services. The only possible place in the Agency of Human Services you might find such applications would be the Department of Health but we do not have their records here in the Central Office.
