Pursuant to Wisconsin's Open Records act, I hereby request the following records:
Debra L. Poulsen, DOB 05/25/1960, is requesting copies of any letters, emails, notes - any correspondence - received from the County or City of Kenosha - regarding Debra L. Poulsen, from 2010 to the present. Debra is requesting any information that has been filtered out to the City of Elkhorn and County of Walworth regarding Debra L. Poulsen. Debra L. Poulsen is requesting who this information was sent to and why. Debra L. Poulsen is requesting any information that has been sent to the County of Walworth Health and Human Services Department during these years regarding Debra L. Poulsen, DOB 05/25/1960, including any notes.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Our agency only has 2 contacts with Debra Poulsen, dob 05/25/1960. The first was on 03/17/2015. One of our deputies was called to assist the City of Elkhorn Police Department with a suspicious persons call. Poulsen was the caller. Law enforcement searched the area and nothing was found.
The second contact was on 02/05/2005. Poulsen received a traffic citation for speeding excess.
