From: Colin Gerber

Hello Jake,

Thank you for the update and my apologies for the delayed response. I am willing to incur the costs and would like to proceed; however, in order to make a payment for the records from the Kansas City PD, my organization requires a completed W9. I've attached a blank form. Would it be possible to have a completed one returned to colin.g@uihi.org? With that in place and an invoice for the total hours, I can submit the payment.

If you have any questions, I can be reached at 612-805-2488.

Thank you,
Colin Gerber

From: Kansas City Police Department

Your written request was received in our records unit
This email will be a much faster method than writing or faxing,

Attached is your invoice and your W9 as requested, the invoice can be paid by phone that is the most efficient method.
Once the invoice is paid I can send you your records

Jake
