A document listing current MDOC guidelines and conditions for supervised and unsupervised probation. This could be a blank Probation Contract or Order, a blank Supervision Order, or an MDOC policy.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Hi Public Records Team,
Thank you so much for your prompt response. Apologies for any miscommunication and inconvenience, but I'm actually looking for conditions of probation as opposed to conditions for parole. I can see in the first document sent it mentions these conditions are provided to the individual to be supervised, but the policy doesn't appear to list the conditions. If you have a sample copy of those conditions of probation, this is what I'm looking for.
