To Whom It May Concern:

This is a request under the Freedom of Information Act. I hereby request the following records:

(1) A copy of any written requests submitted by Primary Investigator David O. Carpenter in reference to CMDRP Gulf War Illness Research Program (GWIRP) Grant Number W81XWH-10-1-1004 asking for additional program funding or asking for an extension of time (with or without costs).

(2) The tier-1 peer review output on Dr. Carpenter's GWIRP Proposal Number GW093066 that was supplied to the GWIRP Integration Panel when they recommended this Innovative Treatment Evaluation Award (ITEA) for funding.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest due to the recent information widely reported on about this ITEA grant funding a clinical trial on a controversial Scientology detoxification program. For more information, please see:

Additionally, requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

Sincerely,

Ms. R. M. Seibert

Ms. Seibert,

This is the first I have seen your request. Did you send it to someone else or this mailbox?

Thank you for the response Sandra.

Since I'm using the automated filing service at MuckRock.com, I don't have direct access to email address that was used. But based on how the system seems to work, I think it's safe to assume that yes it did go to this same mailbox. However, this isn't a presssing request and I can gladly wait for a proper response. So please feel free to take what additional time is needed and adjust the date received however you see fit.

With gratitude,

Ms. R. M. Seibert
