Pursuant to the California Public Records Act, California Government Code
Section 6250, et al., as well as the District Administrative Procedure 3300, I hereby request the following records:

1. The number of (AP 5530.3 (d)) Student Statement of Grievance Forms filed for the 2016-2017, 2017-2018, 2018-2019, 2019-2020, and 2020-2021 academic school years.
For example:
There were V (number) of Student Statement of Grievance Forms filed for the 2016-2017 school year.
There were W (number) of Student Statement of Grievance Forms filed for the 2017-2018 school year.
There were X (number) of Student Statement of Grievance Forms filed for the 2018-2019 school year.
There were Y (number) of Student Statement of Grievance Forms filed for the 2019-2020 school year.
There were Z (number) of Student Statement of Grievance Forms filed for the 2020-2021 school year.

2. The number of (AP 5530.5 (a)) Requests for Grievance Hearing filed for the 2016-2017, 2017-2018, 2018-2019, 2019-2020, and 2020-2021 academic school years.
For example:

5. The number of any (AP 5530.8) appeals relating to a Grievance Hearing Committee decision for the 2016-2017, 2017-2018, 2018-2019, 2019-2020, and 2020-2021 academic school years.
For example:

There were W (number) of appeals related to a Grievance Hearing Committee decision for the 2017-2018 school year.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
