To Whom It May Concern:

Pursuant to the South Carolina Freedom of Information Act, I hereby request the following records:

Data use agreements, data extract specifications (e.g., a list of data fields the state provides), and fee schedule / recurring fees paid by any third party data provider (e.g. Symphony Health Solutions, IMS Health / QuintilesIMS, Wolters Kluwer, and/or Data Niche Associates) related to pharmacy claims data used by the state for invoicing drug manufacturers rebates related to any of the following programs: the Medicaid Drug Rebate Program, state pharmaceutical assistance programs (i.e. SPAP), or Aids Drug Assistance Programs (i.e. ADAP).

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
