Pursuant to the The Missouri Sunshine Law, I hereby request the following records:

I am requesting a log of all incidents where force was used by officers from January 1, 2015 to the present. Please include the following:

-Event number, report number, date and time of incident
-Address of incident, ZIP code of incident, community or beat, RD Number, CB Number
-Officer’s name, Officer’s badge number, Police unit, Officer’s sex, Officer’s race, On-duty or off-duty
-CIT or mental health-related indicator
-Type of Resistance
-Subject’s sex, Subject’s race, Subject’s age, weapon used, Subject’s condition, fatal or non-fatal; and all other available data.
Please provide this data in a spreadsheet or csv format.

I am also requesting any relevant data dictionary, glossary, or other document that explains or identifies the column headings used in the aforementioned log, and any numerical or other codes used to designate type of force or type of situation.

I’m making this request as a journalism student at Northwestern University working on a project for publication with MindSite News, a not-for-profit journalism organization that works with students and partner news organizations to gather, study and disseminate data. I hereby request a fee waiver, as the request is being made for non-commercial purposes with the intention of journalism research in the public interest. If a fee waiver is not granted, I am willing to pay fees for this request up to a maximum of $10. If you estimate that the fees will exceed this limit, please inform me first. Because we are a journalism organization working in the public interest of citizens, I appreciate anything you can do to expedite this request.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
